Getting Started is Easy
Thank you for choosing Women's HealthFirst for your OB/GYN care. We have four convenient locations across Chicago's Northwest suburbs, and we want your first visit to go as smoothly as possible.
New patients can get a head start by downloading, completing, and bringing their registration forms to their first appointment. Established patients can update their information at any time through the patient portal. If you need help accessing the portal or have questions about any of your forms, our team is happy to help — just call us at (847) 808-8884.
What to Bring to Your First Visit
- Completed patient registration forms (downloaded and filled out in advance)
- Your insurance card and a valid photo ID
- A list of your current medications and dosages
- Any diagnostic imaging, films, or prior test results ordered by another provider
- Names and contact information for any other physicians involved in your care
- Any questions or concerns you would like to discuss with your provider

Yes, if you are an established patient. Log in to your athenahealth patient portal to review and update your health information at any time. New patients can type directly into the downloadable PDF forms before printing, or complete them by hand and bring them to their first visit.
No. If you have updated your information through the portal ahead of your visit, our team will have it on file. You do not need to bring paper forms.
Standard records requests are typically processed within 10 to 14 business days, depending on the nature and volume of the request. If you have an urgent need — such as an upcoming specialist appointment or surgery — let us know when you call, and we will do our best to accommodate your timeline.
